When necessary to accomplish the business and affairs of the AS, employees, elected representatives, and volunteers may drive as part of their job or volunteer duties. Before driving on AS business, all drivers must receive supervisor approval and meet the requirements set forth in the AS Vehicle Usage Policy.

Request for Driving Authorization Process for Employees

  1. The prospective driver's supervisor initiates the process by submitting their portion of the AS Request for Authorization to Drive Packet on UKG (hamburger menu > Team HR > Forms > Other Forms).
  2. The prospective driver completes their portion of the form.
  3. The risk Manager reviews the form and initiates a driving record review.
  4. If the prospective driver's driving record meets the standards described in the Vehicle Usage Policy, then Defensive Driver Training will be assigned to the employee in CSU Learn. They should complete the training as soon as possible.
  5. Once the above steps have been successfully completed, the driver and their supervisor will receive an email notification that they are approved to drive on AS business. The notification email will include if the employee is allowed to use their personal vehicle on AS business.

Request for Authorization to Drive Personal Vehicles for Employees

This should be used for drivers who are already approved to drive AS vehicles but now need authorization to drive their personal vehicle(s) for work purposes. New drivers can gain approval for driving AS and personal vehicles simultaneously using the initial approval process above.

  1. The prospective driver's supervisor initiates the process by submitting their portion of the Request for Authorization to Drive Personal Vehicles on UKG (hamburger menu > My HR > Forms > Other Forms).
  2. The prospective driver completes their portion of the form.
  3. The risk Manager reviews and approves the form.
  4. Once the above steps have been successfully completed, the driver and their supervisor will receive an email notification that they are approved to drive personal vehicles on AS business.

Request for Driving Authorization Process for Volunteers, Interns, and Elected Representatives

  1. The prospective driver's supervisor initiates the process by emailing the AS Risk Manager and requesting they begin the process. The Risk Manager will send a form to the supervisor to be completed via Adobe Sign.
  2. After the supervisor has completed their portion it will go to the prospective driver. The prospective driver completes their portion of the form.
  3. The risk Manager reviews the form and initiates a driving record review.
  4. If the prospective driver's driving record meets the standards described in the Vehicle Usage Policy, then Defensive Driver Training will be assigned to the employee in CSU Learn. They should complete the training as soon as possible.
  5. Once the above criteria have been successfully completed, the driver and their supervisor will receive an email notification that they are approved to drive on AS business. The notification email will include if the employee is allowed to use their personal vehicle on AS business.
